TÓM TẮT
Viêm màng não - rễ tuỷ tăng bạch cầu ái toan do ký sinh trùng gây tổn thương tại hệ thống thần kinh trung ương tuy không phổ biến nhưng có thể để lại hậu quả nặng nề nếu như không phát hiện sớm và điều trị kịp thời. Bệnh có xu hướng ngày càng tăng lên do sự giao lưu giữa các vùng ngày càng thuận tiện hơn.
Mục tiêu: Nghiên cứu lâm sàng và điều trị 2 ca bệnh.
Phương pháp: Mô tả ca bệnh.
Kết quả: Bệnh nhân thứ nhất: nữ, 2 tuổi, xuất hiện nôn, sốt dao động 380C đến 400C, liệt mềm hai chân, dịch não tuỷ 1100 bạch cầu/ mm3 trong đó 67% là bạch cầu ái toan. Tìm thấy 2 giun đầu gai từ bệnh phẩm dịch não tuỷ. Bệnh nhân thứ 2: nữ, 25 tháng tuổi, khởi phát sốt 3805C đến 390C, liệt mềm 2 chân, mất phản xạ gân gối, bí tiểu tiện. Dịch não tuỷ 132 bạch cầu/ mm3 trong đó 61% là bạch cầu ái toan. Xét nghiệm huyết thanh dương tính với giun đũa chó. Cả 2 bệnh nhân được chẩn đoán kịp thời và cho kết quả điều trị tốt.
Kết luận: Qua 2 trường hợp viêm màng não - rễ tuỷ do ký sinh trùng chúng tôi thấy biểu hiện triệu chứng lâm sàng do tổn thương màng não - rễ tuỷ gây nên, xét nghiệm dịch não tuỷ có tăng bạch cầu ái toan rất có giá trị, tìm thấy kí sinh trùng hoặc phản ứng huyết thanh học dương tính sẽ củng cố cho chẩn đoán, điều trị sớm sẽ cho kết quả tốt.

SUMMARY
EOSINOPHILIC MENINGITIS AND RADICULOMYELITIS DUE TO NEOMATODE: TWO CASES REPORT
Introduction: Eosinophilic meningitis and radiculomyelitis include all the condition of meningitis and/or radiculomyelitis associated with eosinophilia in the cerebrospinal fruit.
Objective: To study clinical and treatment of 2 cases
Method: We report 2 cases
Results: The first patient was a 2 year-old who had vomiting, flacid paralysis of inferior limbs, fever with temperature usually reaching a maximum of 380 C to 400 C in the early 3 weeks of disease. Examination of the cerebrospinal fluid revealed 1100 leucocytes per mm3 of which 67 per cent were eosinophils. Two actively moving nematode, Gnathostomaspinigenum, were recovered prom the first specimen.
The other was a 25 months old who had feve with temperatures of 38o5 to 390, flacid paralysis of 2 inferior limbs, decreased deep tendon reflexes, urine reserve (+). Examination of the cerebrospinal fluid revealed 132 leucocytes per mm3 of which 61% were eosinofils. ELISA : positive with dog roundworms (toxocariasis)
Conclusion: This review and case study analysis provides the clinician with a critically established set of differential diagnoses and a concise definition of eosinophilic meningitis and radiculomyelolitis due to nematodes in the central nervous systems that may assist the physician in the evaluation of patients presenting with eosinophils in the cerebrospinal fluid.
